The platform captures audio from the session, processes it with AI, and produces a **complete chart note for the visit** — ready for review, editing, and sign-off like any other note in the patient chart. The generated note includes **only clinically relevant information**. Small talk, social chatter, and other **non-clinical conversation** from the visit are **not included** in the chart note — even though the full encounter is still captured in the recording and transcript. AutoChart is designed for in-room or telehealth encounters where you want documentation generated from the conversation rather than typed or dictated from scratch. See [Chart Notes](/clinical-platform/chart-notes/index.md) for how drafts, templates, signing, and edit history work once AutoChart output is in the chart. ## Recording a Visit Start an AutoChart session from a patient’s chart when you’re ready to document the encounter. The session **records the full visit** as audio while you see the patient. **Pause and resume** as needed — for interruptions, side conversations, or breaks in the exam. AutoChart **saves all audio recordings** as you go, so a dropped Wi‑Fi connection, app refresh, or a device running low on battery does **not** wipe the whole session. When you’re back online or on a charged device, pick up where you left off. When the visit is complete, **finalize** the AutoChart session to generate the chart note content from the recording. ## Transcripts Every AutoChart session includes a **transcript of the visit audio** — the full conversation as spoken, including non-clinical content. The transcript is available alongside the generated note so providers and staff can verify what was said, spot-check AI output against the source conversation, or reference specific details from the encounter without replaying the full recording. ## Templates When you finalize an AutoChart session, you **select a chart note template** to put the generated content into. Use one of the **prebuilt chart note templates** included in the platform, or one of your **[custom chart note templates](/clinical-platform/chart-notes/#templates/index.md)** — either way, the output follows a defined **structure and sections**, and maps content into the **fields and macros** you use for manual charting. Templates are optional but recommended — they align AutoChart output with how your team documents visits today instead of producing a generic block of text. ## From AutoChart to Chart Note After you choose a template and finalize, AutoChart creates a **draft chart note** in the patient chart. From that point forward it behaves like **any other drafted chart note**: - **Edit** content before signing - **Sign** when the provider is satisfied with the documentation - **Reassign** the draft to another user — for example a scribe or MA — for completion before provider review Drafts **auto-save** and remain drafts until signed. See [Chart Notes](/clinical-platform/chart-notes/#drafts-signing-and-reassignment/index.md) for the full draft, signing, and reassignment workflow. ## AutoChart Sessions AutoChart has its own dedicated area in the platform where you can **track sessions for a patient** — both **active** recordings in progress and **completed** sessions that have been finalized or are awaiting follow-up. This gives you a clear history of which visits were captured with AutoChart and where each session stands in the documentation workflow. ## Permissions Using AutoChart requires explicit permission. Admins grant **Can Use AutoChart** to individual users from the **Users** page when inviting someone new or editing an existing team member. Users without this permission cannot start or manage AutoChart sessions.
